Paper/Pencil Assessments: A Two-Part Process for Manual Score Entry

 

The paper/pencil option requires a two-part process. Follow the instructions below:

 

Part 1: Prepare Your Materials and Assess Your Students 

 

  • Identify intervention students - Use your Universal Screener results to determine which students qualify for intervention support. (You should only give the Phonemic Awareness Screener for Intervention™ (PASI) and/or Phonics Screener for Intervention™ (PSI) to those who you’ve identified.)
  • Select assessment and starting skill - Choose the appropriate assessment and skill level based on the student's grade and time of year recommendations chart. 
  • Gather your teacher and student materials - Print and prepare the scoring rubrics and student booklet from your PASI or PSI resource tile.  
  • Set up assessment space - Arrange a quiet area for 1:1 assessment administration with your student. 
  • Administer and score - Complete the assessment with your student, recording their performance on the scoring rubric. 


Part 2: Enter Your Student Scores 

 

Before you begin, watch the Entering Assessment Scores Walkthrough video below for step-by-step guidance on how to add your student’s assessment scores into 95 Literacy Intervention System (95 LIS™).



Finding Your Student and Recording Their Scores  

 

  • Once logged into the One95™ platform, click the Students page on the left-hand navigation panel. 



  • On the Students page, use the filter options to find the students you’re associated with, or simply search for the student you're assessing by typing their name in the search bar. Once you find your student, click View Details under their name to access the digital assessments. 
     


  • Complete the Actions panel by selecting the assessment given and entering the assessment date, assessor, form, the starting skill you used for your scoring rubric, then click START ASSESSMENT. 



  • Record the student's answers from your scoring rubric using the scoring tools.  



Next Steps After Entering a Student's Score 

 

  • Select STOP when all scores have been transferred. 



  • When testing is complete, choose to either Hold or Save the assessment: 
    • Hold - allows future edits 
    • Save - finalizes results—no further edits possible 



Repeat the Process! 

 

Now that you've entered a paper/pencil score for one of your students, repeat the process for each assessed student, ensuring all scores are recorded in 95 LIS.
